Field Trip Group Assignment Form
 The field trip group assignment form [PDF] must be completed by all groups participating in the field trip program. The form is used to assign students to the expert and interdisciplinary groups as detailed in the exhibition guides.
This form must be completed and returned to the museum one week before your field trip. This will enable us to prepare materials in advance and will help to make your visit go smoothly. Forms may either be faxed to 202-334-1548 or e-mailed to the Field Trip Coordinator. If you have questions, please call the Field Trip Coordinator at 202-334-1201.
